Evergreen is pleased to announce a partnership with the Stand By Me Program offered by Connecticut Hospice, a participant in Medicare’s GUIDE (Guiding an Improved Dementia Experience) Model. We’ve prepared a list of Questions and Answers to help you take advantage of this valuable program.
What is GUIDE?
The GUIDE Model aims to support people with dementia and their unpaid caregivers. It offers a comprehensive package of services designed to improve quality of life. It pays for the coordination of services, education about dementia, and the support people living with dementia often need to remain in their homes safely.
Why was GUIDE established?
To alleviate caregiver burden. Patients who have a primary caregiver are eligible for GUIDE Model respite services provided in the patient’s home, in a 24 hour facility, or in an adult day center like Evergreen to help you take a break when needed.
How does GUIDE work?
The GUIDE model requires contracting with a Partner Organization (in our case, Connecticut Hospice, d/b/a Stand By Me) to provide in-home respite services. However, participants have the option to make respite available at Evergreen.
How much is the benefit?
$2,700 for the period of July 1, 2025-June 30, 2026. This amount covers 25 free visits at Evergreen Daytime Senior Care, offering engaging activities and social interaction. This annual benefit is renewable.
Are there any restrictions?
Your loved one must receive written approval from CMS (Centers for Medicare/Medicaid Services) before any GUIDE services can be performed.
